【摘要】目的:探讨血清肿瘤坏死因子α(tumor necrosis factorα, TNFα)、血管内皮生长因子(vascular endothelial growth factor, VEGF)水平与糖尿病视网膜病变的关系。
方法:将108例糖尿病患者分为糖尿病视网膜病变组(DR,60例)、非糖尿病视网膜病变组(NDR,48例),DR组又分为非增生性糖尿病视网膜病变亚组(NPDR,34例)、增生性糖尿病视网膜病变亚组(PDR,26例)。健康对照组40例。采用ELISA法检测血清TNFα和VEGF水平,分析对比。
结果:糖尿病患者血清TNFα和VEGF水平与健康对照组比较,有显著性差异(P<0.01)。DR组血清TNFα和VEGF水平显著高于NDR组(P<0.01);PDR组血清TNFα和VEGF水平显著高于NPDR组(P<0.05)。
结论:血清TNFα和VEGF可能参与了DR的发生、发展,血清TNFα、VEGF水平可反映DR病情严重程度。

Relationship among TNFα, VEGF and diabetic retinopathy

Abstract AIM: To explore the relation among tumor necrosis factorα(TNFα),vascular endothelial growth factor(VEGF) and diabetic retinopathy (DR). METHODS: Totally 108 diabetics were divided into diabetic retinopathy (DR) group (60 cases), nondiabetic retinopathy (NDR) group (48 cases), diabetic retinopathy+ nonproliferative diabetic retinopathy (DR+NPDR) group (34 cases) and proliferative diabetic retinopathy (PDR) group (26 cases). Other 40 normal persons were as controls. The level of serum TNFα and VEGF were measured by ELISA and the results were compared and analyzed. RESULTS: The level of serum TNFα and VEGF were higher significantly in patients with diabetes than those in controls (P<0.01), in DR group than those in NDR group (P<0.01), in PDR group than those in NPDR group (P<0.05). CONCLUSION: The serum TNFα and VEGF may participate in the occurrence and development of DR, and be related to the seriousness of DR. 0引言 糖尿病视网膜病变(diabetic retinopathy, DR)是糖尿病常见的并发症,是影响糖尿病患者生活质量的重要原因之一。DR的发生可能与白细胞流变性异常、血管内皮细胞损伤及凝血机制异常有关[1,2]。我们检测了DR患者血清肿瘤坏死因子α(TNFα)、血管内皮生长因子(VEGF)水平,旨在探讨TNFα、VEGF与DR的关系。
1对象和方法
1.1对象 200306/200712 II型糖尿病患者108例,均符合WHO诊断标准。糖尿病视网膜病变组(diabetic retinopathy group, DR):60例,男32例,女28例,年龄43~78(平均60.5±17.0)岁,病程5~20a,所有患者未用抗凝药物。DR组根据DR类型分为两个亚组,非增生性糖尿病视网膜病变亚组(nonproliferative diabetic retinopathy, NPDR)34例,男18例,女16例;增生性糖尿病视网膜病变亚组(proliferative diabetic retinopathy, PDR)26例,男14例,女12例。DR诊断依据全国眼底病学组制定的糖尿病视网膜病变诊断标准,根据眼底检查和眼底荧光血管造影确诊。患有糖尿病但无DR者,非糖尿病视网膜病变组(nondiabetic retinopathy, NDR)48例,男24例,女24例,年龄42~75(平均61.5±15.2)岁,病程5~15a。对照组40例为同期体检健康者,年龄39~75(平均58.5±16.2)岁,男21例,女19例,均无糖尿病家族史,空腹糖耐量正常。
1.2方法 所有受检者均于清晨采集空腹静脉血5mL,离心后将血清置于80℃冷冻保存,采用ELISA法同时测定TNFα和VEGF,试剂盒购自上海太阳生物公司。严格按试剂盒说明书操作。

统计学处理:SPSS10.0软件,检测结果以±s表示,数据间比较采用t检验,检验水准α=0.05。
2结果 糖尿病患者血清TNFα和VEGF水平与健康对照组比较,存在统计性差异(P<0.01)。DR组血清TNFα和VEGF水平显著高于NDR组(P<0.01);PDR组血清TNFα和VEGF水平显著高于NPDR组(P<0.05,表1)。
