【摘要】 目的：探讨一定大小的乳化硅油小滴能否影响视网膜色素上皮（retinal pigment epithelial , RPE）细胞的吞噬能力。方法：将硅油挤压通过一种微孔滤膜制成大小相似的小滴。用酞箐蓝标记硅油小滴。培养的人RPE细胞与硅油小滴，伴或不伴有血清，共同孵育24h。被细胞内吞的小滴在显微镜下计数。测量人RPE细胞在体外吞噬硅油小滴的数量。结果：挤压通过这种微孔膜制成的硅油小滴至少稳定5d，硅油小滴的直径为4.25±0.77μm。乳化硅油浓度不同，RPE细胞吞噬硅油小滴的数量有明显剂量依赖性，随浓度增高，吞噬数量也增加。加有血清的硅油小滴使RPE细胞的吞噬能力进一步增强。结论：硅油小滴能刺激RPE细胞的吞噬能力，含有血清时对RPE细胞的吞噬能力有进一步的刺激作用。

Effects of vesicles of silicone oil on phagocytosis of human retinal pigment epithelial cells in vitro

Abstract AIM: To investigate whether the phagocytosis potentiality of retinal pigment epithelial (RPE) cells can be influenced by emulsified silicone oil droplets with a certain vesicle size. METHODS: Similarly sized vesicles of silicone oil were produced by extrusion of oil through a micro-pore filter membrane. Vesicles of silicone oil were labeled and monitored with phthalocyanine blue dye. Cultured human RPE cells were incubated with labeled silicone oil vesicles and with or without serum for 24 hours. The numbers of the vesicles engulfed in the RPE cells were counted under a microscope. The main outcome measure was the ability of RPE cells to phagocytize the vesicle numbers of silicone oil in vitro. RESULTS: Extruding silicone oil through the membrane resulted in the production of vesicles that remained stable for at least 5 days. The mean vesicle diameter was 4.25±0.77μm. Vesicles of silicone oil had a dose-dependent influence on RPE cell phagocytosis. The more the concentration of the vesicles, the more phagocytosis the cells did. Phagocytosis of the vesicles by cultured RPE cells was also stimulated by serum added in the culture medium.CONCLUSION: These in vitro data show that vesicles of silicone oil are stable once emulsified. Vesicles of silicone oil can stimulate the ability of RPE cell phagocytosis. The serum can further stimulate the ability of the phagocytosis.

硅油又称为聚二甲基硅氧烷，理化性质稳定。没有生物毒性、透光和屈光性好、粘稠度高、能限制玻璃体腔内的增生细胞和生化介质的移动，可能限制增生性玻璃体视网膜病变，并且具有机械性抑制增生膜的牵引作用，能有效封闭裂孔。目前硅油广泛用于治疗复杂孔源性视网膜脱离，特别是复杂视网膜增殖性疾病。但眼内长期放置硅油可引起许多并发症，其中硅油乳化是较严重的一种。硅油乳化是指小而稳定的硅油小滴与玻璃体内硅油大泡分离并扩散进入眼组织的一种过程。此时，乳化硅油对限制玻璃体腔内的增生细胞和生化介质移动的能力降低，封闭裂孔的能力也降低。在视网膜表面的一些地方，硅油小滴黏附紧密，很难用机械的方法将其冲洗干净。张少冲等[1]对玻璃体腔注射硅油患者的这些部位进行了视网膜光学相干断层扫描(optical coherence tomography, OCT)，发现均有视网膜下膜的形成，视网膜色素上皮细胞也有增殖。乳化硅油可能刺激膜形成和随后的增生性玻璃体视网膜病变（proliferative vitreoretinopathy, PVR）反应。吞噬细胞可能被硅油小滴活化，即吞噬等能力增强，并吞噬一定大小的小滴。因此我们在体外用人眼视网膜色素上皮（RPE）细胞分析硅油小滴对其吞噬活性的影响。
