【摘要】 目的:探讨印度式手法小切口白内障摘除术的临床实用性。方法:实验组46眼(46例)行手法小切口白内障摘除术,对照组52眼(52例)行超声乳化白内障吸除术,均联合人工晶状体植入术,比较两组术后视力、手术源性散光、并发症差异。结果:术后3mo,实验组平均手术源性散光为-0.53±0.14D,而对照组为-0.48±0.10D,差异无显著性(P >0.05)。两组术后3mo视力分别为0.71±0.14和0.72±0.12,差别无统计学意义(P >0.05)。结论:手法小切口白内障摘除术是一种简单易行、学习曲线短、效果理想的白内障手术方式。

Clinical observation of manual small incision cataract surgery and intraocular lens implantation

Abstract AIM: To evaluate the clinical results of manual small incision cataract surgery. METHODS: In the case-controlled study, the experiment group included 46 eyes (46 patients) undergone small incision cataract surgery, and 52 eyes (52 patients) undergone phacoemulsification were designated as controls. Postoperative visual acuity, mean surgical induced astigmatism (SIA) and complications were compared between the two groups. RESULTS: The mean SIA was -0.53±0.14D in the experiment group and -0.48±0.10D in controls at postoperative 3 months, with no statistical differences (P >0.05). Postoperative visual acuity was 0.71±0.14 in the experiment group and 0.72±0.12 in controls at postoperative 3 months, with no statistical differences (P >0.05). CONCLUSION: The study suggests that manual small cataract surgery can obtain the same mean SIA and postoperative visual acuity as phacoemulsification.

自1967年Kelman[1]首次报道超声乳化白内障吸除术(Phacoemulsification,Phaco)以来,这项技术经过近40a的发展已经相当成熟了,具有损伤小、效果好的优点。然而,超声乳化技术也有它的缺点,如设备昂贵、手术价格高、受晶状体核硬度的限制,而且学习曲线长。为此,全世界的眼科医生不断探索新的手术方式。自1983年Gerald T. Keener[2]首次报道手法小切口白内障摘除术(manual small incision cataract surgery,MSICS)以来,这个手术方法已经日趋成熟,我们到印度Aravind眼科医院进修学习MSICS,回国后在我院成功开展这项手术,取得良好的效果,现将结果报告如下。
