【摘要】 目的:研究LASIK术矫治1.00D的等效球镜度数时角膜前表面屈光力发生的变化。
方法:以角膜地形图所测得的角膜前表面屈光力(K值)为标准,角膜前表面屈光力变化量ΔK= K术前K术后,按矫治等效球镜度数(SE矫治)将患者分成低、中度近视(SE矫治≤ 6.00D)及高度近视(SE矫治> 6.00D)两组,计算ΔK/SE矫治,并与由经验公式推导出的0.8进行统计学分析。
结果:当矫治1.00D的等效球镜度数时,总体研究对象:低、中度近视组和高度近视组的角膜前表面屈光力分别变化0.89±0.24D,0.94±0.28D和0.83±0.18D;切削的角膜组织分别为11.46±0.76μm,11.87±0.65μm和11.08±0.65μm;角膜的切削量与矫治等效球镜度数呈负相关关系。
结论:LASIK术后低、中度近视的角膜前表面屈光力变化大于高度近视。

Relationship between the change of anterior corneal refractive power and spherical equivalent degree after LASIK

AbstractAIM: To evaluate the change of anterior corneal refractive power as spherical equivalent degree (SE) changes 1D in LASIK.
METHODS: Anterior corneal refractive power (K value) was measured by corneal topography. The change of anterior corneal refractive power ΔK=KpreKpost. The patients were divided into two groups based on SE: low to moderate myopia group (SE≤ 6.00D) and high myopia group (SE> 6.00D). We calculated ΔK/SE and analyzed the difference between it and 0.8 which was conducted from empirical formula.
RESULTS: When spherical equivalent degree changes 1.00D, the change of anterior corneal refractive power of all, low to moderate myopia and high myopia were 0.89±0.24D, 0.94±0.28D and 0.83±0.18D, respectively. The cut corneal tissue of two groups was 11.46±0.76μm, 11.87±0.65μm and 11.08±0.65μm. There was a negative correlation between the quality of cut corneal tissue and spherical equivalent degree.
CONCLUSION: The anterior corneal refractive power changes more in low to moderate myopia than that in high myopia after LASIK.

LASIK作为一种安全、有效、预测性强的手术,其术后效果是患者和手术医生非常关心的问题。有研究表明,术后角膜曲率是影响视觉质量及屈光稳定性的重要因素[13],因此客观准确的预测术后角膜屈光力,可为手术设计及预测术后效果提供重要参考。本研究探讨角膜前表面屈光力变化与矫治等效球镜度数的关系,结果报告如下。
1对象和方法
1.1对象 随机选择2005 06/200710于中国医科大学附属第一医院近视眼治疗中心LASIK术后超过2mo的患者52例104眼,其中男18例36眼,女34例68眼,年龄17~46(平均25.31±6.82)岁,术前等效球镜度数为1.25~15.00D(平均 6.62±2.59D),手术矫治等效球镜度数(SE矫治)为1.50~13.25D(平均 6.56±2.07D),术后裸眼视力均≥1.0,无屈光回退。按SE矫治将患者分为低、中度近视(SE矫治≤ 6.00D)组25例50眼及高度近视组(SE矫治> 6.00D)27例54眼。所有患者手术前后均无角膜活动性炎症,无翼状胬肉,术前均无其他屈光手术史。
