4.4 激光治疗
1960 年代出现的激光封孔，成功率高，损害小，且激光可改变视网膜色素上皮的屏障作用，达到促进视网膜下液吸收的功效，从而保护和改善视力。Ohana等[19] (1998年) 利用眼外激光光凝联合气—液交换成功地治疗了黄斑裂孔病例。与以往黄斑裂孔光疑治疗不同的是，他们利用氪黄激光对黄斑裂孔底部RPE进行轻微的环形光凝，目的是通过轻度光凝激发RPE的增生。15例复发的黄斑孔中13例闭合。Ikuno等[20]利用氩绿激光光凝黄斑裂孔底部RPE，13例再张开裂孔的患者中12例闭合。玻璃体手术当中也可适当辅助以眼内光凝。Margherio 等[21]采用氩绿激光对黄斑裂孔底部RPE 进行单点轻度光凝，治疗了3例未能行内界膜和黄斑前膜剥离的4 期黄斑裂孔患者。3例裂孔均闭合。2例术后视力恢复至20/25(0.8)。1例裂孔闭合但视力无改善。术后中心凹下相应于光凝点的位置，出现RPE 的增生。可见对于某些黄斑裂孔病例，适当配合激光治疗，有利于裂孔的闭合，同时由于光凝的程度较轻，对术后视力的恢复通常不会产生不良影响。迄今尚无光凝诱发脉络膜新生血管膜的报道。目前激光治疗可用氩绿激光，氦红激光或倍频黄绿激光，最好不用氩蓝绿光，蓝光易被黄斑叶黄素吸收造成视网膜损伤，临床上氩激光应用较多。严盛枫等[22]认为，激光封闭黄斑裂孔可以改变色素上皮的屏障作用，达到促进视网膜下液吸收功效，从而保护或改善视力。激光治疗应合理应用激光参数，如波长、能量、光斑大小、作用时间，否则易出现激光并发症。
因此，鉴于外伤性黄斑裂孔的临床特点和转归过程，大多数学者倾向于先观察3～4mo，若无自发闭合考虑激光手术，单纯PPV 联合注气术或ILMP术。在黄斑裂孔治疗中重要的是术前确定玻璃体视网膜交界面状态，以决定手术方式。因此，术前OCT检查非常必要。
