ultrasound biomicroscopy and transmission electron microscopy study of a case of true exfoliation of the lens capsule with angle closure glaucoma

True exfoliation or lamellar delamination of the lens capsule is a rare disorder in which the lens capsule is thickened and the superficial portion of the lens capsule splits from the deeper layer and extends into the anterior chamber.The pathogenesis of this disorder is not clear,but intense infrared radiation has been thought to be the main causative factor.There were reports about the True exfoliation with angle closure glaucoma.We describe a patient bilateral delamination of the anterior lens capsule.with attact period of angle closure galucoma in the right eye,and eclipse period in the left eye.After the patient received Phoco+IOL implication in the left eye,we examined the bilateral lens capsule with Ultrasound Biomicroscopy,and the left lens capsule with transmission electron microscopy,demonstrated a lamellar separation of the anterior portion of the lens capsule,and interesting phenomenon of multilamellar of the capsule,which was not reported before. |
