【摘要】 目的:探讨糖尿病视网膜病变(diabetic retinopathy,DR)患者血浆内皮素1(endothelin1,ET1)、降钙素基因相关肽(calcium gene related peptide,CGRP)的水平变化,以阐明其在糖尿病并发视网膜病变中的作用。方法:应用放射免疫分析(RIA)检测123例DR患者和157例对照组(未并发DR)患者血浆ET1,CGRP 的浓度。结果:病例组的患者的血浆ET1(186.2±44.0ng/L),CGRP(46.4±13.3ng/L)与对照组(135.3±28.1ng/L)和(73.2±19.6ng/L)相比较,差异均有显著性意义(P<0.01) 。而非增殖型与增殖型患者对照比较亦有显著性差异(P<0.05)。增殖型患者血浆ET1,CGRP水平呈负相关(r=0.679,P<0.05)。结论:ET1和CGRP在DR发病过程中可能有着重要的病理作用,并且与DR病变严重程度有着一定的关联,血浆含量检测有助于判断病情的程度。

Clinical significance of changes of ET1/CGRP contents in patients with diabetic retinopathy

AbstractAIM: To illustrate the effects of endothelin1 (ET1) and calcitonin gene related peptide (CGRP) in patients with diabetic retinopathy (DR) through investigation of the changes of the plasma levels of ET1 and CGRP.METHODS: Plasma ET1 and CGRP concentrations were measured with RIA in 123 patients with DR and 157 controls.RESULTS: Plasma ET1(186.2±44.0ng/L) and CGRP (46.4±13.3ng/L) levels in the patients with DR were significantly different from those in the controls (ET1: 135.3±28.1ng/L, CGRP: 73.3±19.6ng/L, P<0.01). Differences between the levels of these nonproliferative diabetic retinopathy and proliferative diabetic retinopathy were also significant (P<0.05). In patients with proliferative diabetic retinopathy, the plasma ET1 levels were negtively correlated to the plasma CGRP levels (r= 0.679,P<0.05). CONCLUSION: Plasma ET1 and CGRP play important role in the whole pathophysiologic process of DR and correlate to the severity of the disease, measuements of their plasma concentrations help to predict theseverity of the disease. 0引言
血浆内皮素1(endothelins1,ET1)、降钙素基因相关肽(calcium gene related peptide,CGRP)是体内的重要血管活性多肽,二者分泌失衡在许多伴有血管内皮细胞损伤的病理过程中起着重要的作用,诸多学科已经有了较为广泛深入的研究。本文采用放射免疫分析(RIA)测定了160例糖尿病视网膜病变(diabetic retinopathy, DR)患者血浆ET1及CGRP的含量,以期阐述其在DR发病中的作用以及对DR早期、分型诊治的临床意义。
1对象和方法
1.1对象
我院200609/200802内分泌科就诊的2型糖尿病患者共280例(1985年WHO糖尿病诊断标准)。以眼底镜检查联合眼底荧光血管造影(fundus fluoresceinangiography, FFA)检查为诊断DR的金标准,分为病例组(并发DR)123例,对照组(未并发DR)157例。病例组中非增殖型86例,增殖型37例(2002年糖尿病视网膜病变国际临床分型标准[1])。病例组男69例,女54例,平均年龄(62.0±9.7)岁, 平均病程(6.5±3.0)a;对照组男85例,女72例,平均年龄( 56.8±10.3)岁, 平均病程(3.1±2.5)a。年龄、性别分布病例和对照组统计学分析差异无意义,两者均衡可比。平均病程表现为病例组长于对照组, 统计学分析差异有显著性意义(P<0.01)。两组人群均无全身严重性并发症。两组均正规内科治疗控制血糖水平, 并停用可能影响凝血及血液流变学的药物1wk。
1.2方法
空腹抽取肘静脉血2mL, 注入含75g/L EDTA二钠30μL和抑肽酶40μL的试管中,混匀, 4℃。3000rpm离心10min,分离血浆, 放射免疫法(RIA)测定血浆ET1和CGRP的含量。测定仪器为上海原子核研究所日环仪器厂SN 695B型智能放射免疫γ检测仪,试剂为北京东亚免疫生物技术有限公司生产的125I内皮素放射免疫分析试剂盒。 统计学分析:实验数据以±s表示,采用SPSS 11.0软件包进行t检验统计分析。
2结果
病例组血浆ET 1,CGRP含量以及ET1/CGRP比值分别为186.2±43.9ng/L,44.0±13.3ng/L和3.3±2.0。对照组分别为135.3±28.1ng/L,73.2±19.6ng/L和2.3±1.5。病例组血浆ET 1含量升高,CGRP含量降低,ET1/CGRP比值增加,与对照组相比差异具有统计学意义(P<0.01)。病例组中非增殖型患者血浆ET 1,CGRP含量以及ET1/CGRP比值分别为173.4±39.8ng/L,52.4±12.8ng/L和3.0±1.8;增殖型患者血浆ET 1,CGRP含量以及ET1/CGRP比值分别为205.3±49.4ng/L,34.1±10.2ng/L和4.6±2.1。DR程度越严重,血浆ET1含量升高,CGRP含量降低越明显(P<0.05)。增殖型病例血浆ET1与CGRP呈水平负相关(r=0.679,P<0.05),非增殖型病例ET,CGRP水平无直线关系(r=0.148,P>0.05,表1)。 表1 DR患者血浆ET1、CGRP含量(略)
