【摘要】 目的:比较应用Tecnis MF和ReZoom两种多焦点人工晶状体植入术后1mo患者视力及对比敏感度(contrast sensitivity,CS)变化情况。方法:采用同期随机对照研究,选取年龄相关性白内障患者58例(74眼),随机分为两组,ReZoom IOL组( 28例, 28眼)植入人工晶状体ReZoom, Tecnis MF IOL组(30例, 46眼)植入Tecnis MF IOL。术后第1mo检查矫正视力、无眩光及眩光环境下对比敏感度。结果:Tecnis MF近视力优于ReZoom(P<0.05),ReZoom中视力优于Tecnis MF(P<0.05),两者远视力无显著差异。Tecnis MF在空间频率为18c/d和24c/d时对比敏感度优于ReZoom(P<0.05)。两者眩光对比敏感度在各空间频率均无显著差异。结论:Tecnis MF及ReZoom在屈光晶状体手术中,对患者预后的影响各异,因此个性化使用Tecnis MF和 ReZoom IOL为影响患者预后的关键环节。

Clinical study of visual functions after Tecnis MF and ReZoom intraocular lens implantation

Abstract AIM: To investigate the visual quality after implantation of Tecnis MF and ReZoom intraocular lens (IOL) in patients' eyes with cataract. METHODS:IOL implantations were performed in 58 (74 eyes) cataract patients. Among them, 28 patients (28 eyes) were randomly selected as Tecnis ZM900 IOLs group, and 30 patients (46 eyes) were selected as ReZoom group. Eyesight and contrast sensitivity with or without glare were examined 1 month postoperatively. RESULTS: Far eyesight between two groups postoperatively is not different significantly. ReZoom is better in middle sight but Tecnis MF is better in near sight(P<0.05). Without glare, the Tecnis MF group showed significantly higher spatial contrast sensitivity than the ReZoom group in the 18c/d and 24c/d frequencies; with glare, each group showed no significantly difference in all frequencies. CONCLUSION: Tecnis MF and ReZoom IOL are new intraocular lens that was the recent focus clinically. It is the sticking point of choosing IOL for individual. 0引言 随着白内障手术从复明手术向屈光手术的转变,人工晶状体(intraocular lens, IOL)的类型对预后影响日益受到关注。多焦点晶状体就是通过同一个晶状体使患者在不同的距离都能看清物体[1]。多焦点晶状体能在白内障或者屈光晶状体置换后,提供最大可能的减少对眼镜和隐形眼镜的依赖[2]。1987年Keates首次报道植入38例(46眼)多焦点人工晶状体,95%眼视力≥20/40[3],1997年Array第一个获得美国FDA认证的多焦点人工晶状体,ReZoom与Tecnis MF为近几年推出的新型多焦点晶状体。Tecnis MF ZM900多焦点人工晶状体是一种不依赖瞳孔大小的全光学面衍射设计的晶状体;ReZoom是一种能够增强患者中视力的折射型多焦点晶状体(看电脑的视力)。这两种晶状体的植入均能够矫正白内障术后患者的远视力和老视[4,5]。本研究观察白内障患者植入Tecnis MF多焦点人工晶状体及ReZoom 多焦点人工晶状体后视力、对比敏感度(contrast sensitivity,CS)与眩光对比敏感度的差异。
1对象和方法
1.1对象 根据纳入标准收集在我院白内障专科入住的白内障患者58例(74眼)。入选标准:排除眼部外伤手术史;排除高度远视或高度近视,测量人工晶状体度数在15.00~25.00D之间达到正视的病例;排除白内障以外,对通过自动角膜屈率计测出的角膜散光>1.50D的或有其他角膜病变的病例;排除有其他影响视力及对比敏感度的全身性疾病(如糖尿病视网膜病变等)的病例。入选患者随机分为两组,ReZoom IOL组( 28例28眼)植入ReZoom IOL,Tecnis MF IOL组( 30例46眼)植入Tecnis MF IOL。ReZoom组患者年龄34~82(平均55.5±11.7岁),Tecnis MF组患者年龄39~83(平均57.4±10.3岁)。根据SRK T公式计算IOL度数,预留度数为0.00~0.50D。

1.2方法 手术采用Sovereign系列超声乳化仪进行超声乳化白内障摘除。术前由爱尔卡因局部表面麻醉,作3.2mm长颞侧透明角膜自闭切口,前房内注入粘弹剂形成前房并保护角膜内皮。用撕囊镊连续环形撕囊,撕囊口直径大小5.5~6.0mm,水分离,行超声乳化白内障吸除术(能量峰值30%,最大吸力550mmHg),I/A吸除残余皮质,注入粘弹剂,植入人工晶状体于囊袋内,吸除粘弹剂,角膜伤口不缝合。球结膜下注射地塞米松2mg。所有手术由同一操作熟练的医师完成。术后检查Tecnis MF IOL,ReZoom IOL植入术后1mo,分别在明(85cd/m2)暗(4cd/m2)条件下采用国际标准视力表比较远视力(5m),Colenbrander混合对比度卡集比较中视力(40,63cm及100cm),应用Jaeger表比较近视力(30cm),同时应用RM800型对比敏感度仪进行对比敏感度及眩光对比敏感度检查。各空间频率对比敏感度均取对数值。 统计学分析:应用SPSS 11.5软件包进行数据分析。组间采用独立样本t检验,P<0.05被认为差异有显著性。
