3 展望
目前,DLEK手术还存在着一些问题,例如:保存的供体角膜一般是水肿的,而内皮失代偿的患者角膜也是水肿的,即使通过右旋糖酐、升高眼压等方法也很难使供体的角膜厚度达到正常水平,如何更加精确地预测移植片的厚度使之与植床厚度相适应还需要进一步研究。
此外,对于DLEK移植手术的一些设想还需要进一步论证:(1)瓣式DLEK采用更大直径(10~11mm)和更厚的角膜瓣,术中通过向前房内填充气泡,使移植片自动贴附,像口袋式手术那样,不必缝合移植片[11],术后将会获得更好的屈光状态。(2)利用体外培养的CECs进行DLEK手术。(3)利用生物组织黏合剂替代缝线,可以减免缝线所带来的风险。
随着新手术器械的开发和手术技巧的完善,相信不久的将来更加完善的DLEK手术可以在临床上广泛开展。
