【摘要】 目的 探讨高频彩超诊断视网膜中央动脉硬化的临床应用价值。方法 使用高频彩超检查40例眼底动脉硬化患者视网膜中央动脉,分析频谱形态,测量频谱收缩期、舒张期血流速度、阻力指数以及搏动指数。同时与正常组对照。结果 眼底视网膜中央动脉硬化患病组彩色血流信号减少;舒张末期血流速度减慢;收缩期波峰园钝;舒张期血流低平、中断;血管阻力指数和搏动指数明显增大。结论 视网膜中央动脉硬化有其彩色多谱勒血流和频谱异常表现特点。高频彩色多谱勒超声诊断该病有较充分的理论依据,有一定的临床诊断价值。

Study on Clinical Application of High-frequency Color Dopplor Ultrasonoscope in Diagnosing Central Arteriosclerosis of Retina

ABSTRACT Objective To discuss the clinical application of high-frequency Color Dopplor Ultrasonoscope in diagnosing central arteriosclerosis of retina.Methods Examinations with High-frequency Color Doppler Ultrasonoscope were made to the central artery of retina of 40 cases with arteriosclerosis of ocular fundus; the spectrum were analyzed and systolic blood velocity, diastolic blood velocity, resistance index (RI) and pulse index (PI) were measured; a comparison was made with the healthy.Results Less color signals of blood velocity were found in patients with central arteriosclerosis of retina in ocular fundus; the diastolic blood velocity was slow and the spectrum peak of systole was dull; RI and PI were very high.Conclusion High-frequency Color Doppler Ultrasonoscope is of certain value in diagnosing central arteriosclerosis of retina as the Color Doppler images of its blood velocity and spectrum are obviously abnormal.

本文应用高频彩超对40例眼底动脉硬化患者的视网膜中央动脉彩色血流及血流动力学变化与40例健康者进行对比研究。为探讨超声诊断视网膜中央动脉硬化的新途径,现报导如下。
1 对象与方法
1.1 对象 患病组:由本院内科和眼科确诊为眼底动脉硬化患者40例。年龄50-82岁,平均62岁。男27例,女13例。其中患有高血压病23例,糖尿病10例。患者无明显心律失常和心功能障碍。患病最长者30年。对照组:正常中老年人40例。年龄45~70岁,平均58岁,无糖尿病、高血压,心脑血管病。眼底镜检查正常。
1.2 方法 使用aloka-5500型彩超仪。探头频率10-13MHz+谐波2.14~2.5mHz,探查深度35mm~40mm,彩色增益5~6MHz,取样容积1.5mm~2mm,取样标速6cm~8cm/s。患者取仰卧位,轻团双眼,探头置于上眼睑处作横、纵向检查。二维图像观察眼内结构及视神经分布。彩色多谱勒血流(CDFI)观察视神经内视网膜中央动脉血流信号。将取样容积置于视乳头后3mm~5mm处,获取视网膜中央动脉多谱勒血流频谱,测量收缩期最大血流速度S,舒张末期最小血流速度D,阻力指数RI,搏动指数PI。将数据输入电脑。流速单位cm/s。
